What the Job Involves
As part of a dynamic and integrated team of health and social care professionals, you will assist service users in tasks defined within their care plans. You will collaborate with other professionals to minimize risks and enable individuals to remain in their homes. Duties include participating in assessments and reviews, supporting recreational activities, and contributing to the maintenance of records while reporting significant changes. You will work Monday to Friday from 9am to 4pm, remaining flexible to adapt to the evolving needs of the service. Additionally, you will ensure that all operations adhere to City Council policies and health and safety legislation.

- Up-to-date classroom M&H certificate
- Strong communication and teamwork capabilities
